1. Consumer(Client/Patient):
Consumer(Client/Patient), who wishes to use services of Medical and Healthcare Service Provider (MHSP), whether In-Clinic or Online, or host his Personal Health Record (PHR), needs to register on the Platform. In addition, he needs to subscribe to one of the Plans to be able to access his records and avail services of MHSP. In addition, Consumer may need to pay Platform Usage Charges as defined in the plans.

Registration and Usage Process:
Step 1- Go to www.onlinepolyclinic.com and Register as a New User (If not already registered).
Step 2- Choose Plan.
Step 3- Select Doctor under 'My Account'
Step 4- Click on the 'Book Appointment'.
Step 5- Select from online/in-clinic consultation & select the suitable time slot.
2. Medical Doctor and Psychologist:
Medical Doctor and Psychologist, who wishes to use the Platform for offering his services, must register on the Platform. He must also subscribe to a plan before being able to start using it.

Plans for Doctors / Psychologists listed below. They can
create a complete Practice Management System, defining their
Clinic and Online timings slots, viewing and creating Medical
records of their patients, doing consultation (including On-line),
writing Prescription and printing Bills / Certificates. Doctor's fee for Online Consultations will be collected thru payment gateway and credited to Doctor’s a/c after deducting payment G/w charges. Doctor's fee for In-Clinic Consultations should be collected by Doctor in the clinic itself. |

3. Diagnostic Centre:
Diagnostic Centre, who wishes to use the Platform for offering his services, must register on the Platform. He must also subscribe to a plan before being able to start using it.

4. Chemist / Pharmacy:
Chemist / Pharmacy, who wishes to use the Platform for offering his services, must register on the Platform. He must also subscribe to a plan before being able to start using it.
